External link icon at end of clickable labels, new tab, add some missing links

Review clickable texts that take user to external service. Add external link icon at end of clickable text as visual [and clickable] indicator.

• ClinVar Id
• Gene
• Expert Panel [link missing but discussed/known; migration likely needed for affiliation ID]
• Condition [link missing but same as MONDO which is a good column to hide by default]
• MONDO

Note:

• The clickable text doesn't need to be blue this way; icon is the indicator. In fact, force to normal color. Hover with underscore can be kept but all text now black.
• The external link icon should be blue though. The current blue (e.g. easy to see with Gene values) is good. For any additional column, consider external link opportunities and follow this convention
